Please mark classes and fees on the application and send to Billie Leach. Include the amount to be paid with this form. All fees due July 20th, and a return letter will be sent on or around July 21st about miscellaneous information. Enrollment limited. A late fee of $10.00 will be charged for late registration after July 1, 2011.  A $5.00 discount for each application received by June 20, 2011.  All fees are non-refundable after July 20th, 2011.
